Your FAFSA usually tells us everything we need to know to create an award notice. But sometimes we need supplemental information to complete the process.

Forms requested by the Financial Aid Office will be listed on your To-Do List in My.IllinoisState.edu. If a form is not listed on your To-Do List, it is not required by the Financial Aid Office at this time. To check the status of any item, click on the "More" button on your To-Do List. This will show you which documents are needed or have been received, for example.

Important Information about Summer Session 2024 Financial Aid

In order to receive summer 2024 financial aid, you  must complete the 2023-2024 Free Application for Federal Student Aid (FAFSA).

YOU SHOULD COMPLETE THE FOLLOWING APPLICATION IF:

  • You are planning to attend at least half-time during the Summer Session 2024 (6 undergraduate credit hours or 3 graduate credit hours) AND wish to be considered for A Federal Direct Loan (you must have remaining eligibility for summer based on the table below).

The chart below illustrates the maximum amount you may borrow under the Federal Direct Loan Program during the award year, which includes summer.

Combined subsidized and unsubsidized* Federal Direct Loan amount cannot exceed the following award year (fall/spring/summer) maximums:

Dependent Student

1st Year Undergrad (0-29 hours)

2nd Year Undergrad (30-59 hours)

Other Undergrad (60+ hours)

* Interest is charged on unsubsidized loans while the borrower is in school and during the grace period.

If you have borrowed the maximum Federal Direct Loan amount for the 2023-2024 academic year, you will have NO Direct Loan eligibility for the Summer Session unless you have advanced a class level that increases your Direct Loan eligibility.

NOTE:  Parents of a dependent student may request a loan under the Federal Direct PLUS Loan program. The parent borrower must go to the studentaid.gov website and follow the directions for applying for a PLUS loan.

NOTE:  Summer aid will be processed based on the enrolled hours you submit on this application.  Reduction of hours may cause a reduction in financial aid.

Loans

Student Federal Loan Application

If you were not awarded a loan as part of your initial financial aid package and you now want to request a loan, or you were awarded a loan as part of your initial financial aid package but you want to request an increased amount. Check with the Financial Aid Office to determine if you have remaining loan eligibility.

Nursing Loan Personal Data Form

If you have been offered and accepted a Nursing Loan on your Offer Letter. This is not an application for the Nursing Loan; Nursing Loan eligibility is determined by your FAFSA.

Nursing Loan Financial Aid Transcript

This form must be sent to each college/university you have attended in the past to help determine if you qualify for a Federal Nursing Loan while attending Illinois State University.

Private Loan Request - No FAFSA Filed

Complete this form if you have applied for a private/alternative loan, but have not yet completed a FAFSA.

Private Loan Request - Documents Still Needed for Federal Aid

Complete this form if you have applied for a private/alternative loan, but have not yet submitted all financial aid items requested on your To Do List.

Private Loan Request - Federal Loans Not Accepted or Declined

Complete this form if you have applied for a private/alternative loan, but have not yet accepted or have declined federal student loans offered.

Other Forms

Satisfactory Academic Progress Appeal Form

If financial aid eligibility is suspended as a result of failure to meet the completion rate or maximum timeframe standard, use this form to submit an appeal to be reconsidered for Financial Aid. For more information, please review our Satisfactory Academic Progress policy.

Dependent Other Than Spouse/Child Form

If your FAFSA indicates you reported dependents other than your children or spouse who live with you and who receive more than half their support from you, July 1, 2023 through June 30, 2024.

Dependent Child(ren) Verification Form

If your FAFSA indicates you reported a dependent child or children who live with you and who receives more than half their support from you, July 1, 2023 through June 30, 2024.

2023-2024 TEACH Grant Application

If you would like to apply for the Federal TEACH Grant. Undergraduate students may only receive the TEACH grant for their first undergraduate degree; you must be enrolled in a major listed on the application form to be considered.

2023-2024 Independent Proof of Residency Form

To be eligible for an Illinois Student Assistance Commission Monetary Award Program (ISAC MAP Grant), you need to provide proof that you have resided in Illinois continuously since August 15, 2022.
